Debit or Credit card details will need to be entered for all telehealth appointments and funds held at the time of booking as we have a new automated payment system.

Confirmation of all appointments will be needed prior to the appointment via SMS.
We understand that sometimes you need to reschedule appointments. Therefore, we respectfully request that you provide us with as much notice as possible when cancelling. A minimum 2 hour notice is need for GP appointments. And a minimum of 24 hours notice (cancelling/rescheduling) for allied health or speacialists is necessary to avoid cancellation fees. A last minute cancellation or no show fee of either $50 for allied health or $100 for specialists appointments will be charged otherwise.
Please understand that when you forget, cancel, or change your appointment without giving enough notice, we miss the opportunity to fill that appointment time, and patients on our waiting list miss the opportunity to receive services.
IMPORTANT BILLING INFORMATION

Coburg Family Medical Centre is a MIXED billing clinic. If you hold a valid health care/concession card, or is a patient aged 15 years or younger then we will guarantee BULK BILLING for your standard GP appointment.
BULK BILLING for eligible patients will be guaranteed for most of the consultation type for GPs. However, please note there will be certain instances where Bulk Billing cannot be guaranteed such as Iron infusions, procedures etc. There will also be private fee for Specialists, Dental, some Allied Health, and all other Non-GP appointments.
Patients with NO CONCESSION medicare and for all patients who do not hold a valid Medicare Card will incur an out-of-pocket fee for the consultations. We are able to help assist with your medicare claim provided that a physical Debit Card is present. Patients with overseas insurance must bring the physical card to their appointments.
We require full payment on the day of your appointment. The fee is to be paid by EFTPOS or if paid by CASH, the EXACT amount needs to be provided as we do NOT keep money on the premises.
Also, a cancellation fee may apply if you fail to turn up to your appointment or cancel your appointment within 2 hours of appointment booking time for GP consultations and 24 hours for specialists/allied health consultations.
